View and Download Our Intake Materials
What follows is our intake materials. Most people find that completing these forms takes some time and can be emotionally draining. For that reason, we encourage you to complete the forms at our office, so someone will be available to talk to you if necessary. However, we do make the forms available online or by email for your convenience. Completing the forms will give your counselor background information to better understand you and your journey, and may help prepare you for the work of counseling.
Important Information Regarding Communication and Privacy
All communication with clients is strictly confidential. This applies to verbal and written communication and therapy notes. Should you request information be released to a third party, a written release of information will be provided for you to sign. This includes the cooperation waiver included with the intake materials.
Many clients prefer communication via text and email. Please be aware that there is a risk of breaching confidentiality since the identity of the person writing the text or email cannot be verified definitively. For this reason, privacy cannot be completely guaranteed via text and email. Should texting and emails occur, it should be for exchanging information such as appointment times, referrals and other “housekeeping” issues.
In addition, there will be no communication with your counselor through social media sites including Facebook, LinkedIn and Instagram. Your counselor will not respond to friend requests or requests to join a client’s social media platform. Similarly, the counselor will not request a client join her social media platform(s). This is for the client’s protection and helps to maintain confidentiality and a professional relationship.
“Let the word of Christ dwell in you richly, teaching and admonishing one another in all wisdom, singing psalms and hymns and spiritual songs, with thankfulness in your hearts to God. And whatever you do, in word or deed, do everything in the name of the Lord Jesus, giving thanks to God the Father through him.”
